What Benefits Am I on Now?

The first step in benefits planning is to know your current situation.

As part of meeting with a benefits planner or using DB101's Benefits to Work calculator, you can request a Benefits Planning Query (BPQY) from the Social Security Administration.

This brief report offers a wealth of information, including details about your SSDI and SSI benefits, your work history, earnings history, and health coverage.

There are a number of ways to request your BPQY. You can call Social Security's toll free number, 1-800-772-1213, and tell the representative that you would like a BPQY. You can also contact the Area Work Incentive Coordinator in your area. A list of CoordinatorsOffsite Link is available. They will give you the name and telephone number for the field office closest to you. The Work Incentive Liaison or any available representative at that office will be able to assist in getting your BPQY.

You must ask for the BPQY yourself, or provide two separate signed consent forms asking for the release of the BPQY to a third party (e.g. – a representative payee, family member or friend). See the sample consent forms and text provided in Appendices 3 and 4 of the Benefits Planning Query Handbook Version 1.08Adobe Acrobat Document. We recommend that you use that sample text when filling out these forms.
